Turning 18 is a major milestone and can often elicit questions related to the responsibilities of being an adult. At Alta California Regional Center (ACRC), we are here to provide support while navigating into adulthood through end of life.
Clients of ACRC are assigned a designated Service Coordinator. Together, the Service Coordinator, client and family members they wish to include encompass the Planning Team. The Planning Team works together to review all appropriate services and supports. Services and supports are developed by building upon a client’s natural supports while maximizing the use of generic resources, as well as arranging to purchase services and supports that are identified in the client’s Individual Program Plan (IPP). ACRC believes and supports service options that value clients as productive and contributing members in their community.
Some common topics discussed by the Planning Team can include:
- Adult Programs
- Community Integration
- Durable Medical Equipment
- Environmental Accessibility
- Future Planning/Retirement
- Healthy Relationships
- Navigating and Accessing Service Systems
- Respite Services
- Self-Advocacy and Client’s Rights
