The required HCBS Self-Assessment must be completed by Monday, August 31, 2020!

Providers of the below identified service codes must complete the HCBS Self-Assessment to measure their current level of compliance:

  • Residential: 096, 113, 904, 905, 910, 915, 920
  • Day Programs: 028, 055, 063, 505, 510, 515, 855
  • Employment Programs: 950, 954

Accessing the Self-Assessment

  • Creating an account
    • If a representative from your agency has not already received an email with login credentials, please go to the website and click on “Need an account”. This will allow your agency to register and access the required Self-Assessment.
  • Password reset
    • To reset the password for your agency’s account, you can go to the website and click “I forgot my password.”
  • If you experience any technical difficulties on the website, please contact the OIG Help Desk.

When completing the Self-Assessment, remember…

  • Save often to prevent being timed out and losing unsaved responses!
  • Agencies do not already need to be in compliance. There is time to transition.
  • It is crucial to include input from individuals receiving services to ensure services are individualized and person-centered.
  • Any provider that does not complete a self-assessment will receive an onsite assessment.
    • Be sure to carefully answer all questions and sub-questions because incomplete assessments will also be flagged for an onsite assessment.
  • Each person receiving services is unique, and so is each agency.
    • As a result, there are many ways to align services in California to meet the federal requirements. There is no one “right” answer.
