Central Valley Training Center Project 1. CCL licensed day program providing services and supports to clients with behavioral challenges. Clients are those who will be transitioning from an SDC or from a CPP developed facility. Start up funds are for program modifications, trainings, equipment, program supplies, etc. that are specifically related to the needs of the targeted clients population and other enhancements of the program. $50,000
Advanced Kids Project 2. Provide intensive behavioral supports in a client’s current placement to mitigate placement in a more restrictive setting such as an SDC, IMD, MHRC, etc. Start up funds are to assist in the development of this program including training staff, program supplies, etc. $50,000
Kind Connection Project 3. Four bed specialized residential facility for individuals exiting out of the developmental centers or at risk for admission that require specialized medical and behavioral treatment. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $150,000
California Housing Foundation (NPO) Project 4. Utilization of a non-profit housing development corporation for the acquisition and development of an ARFPSHN home. Start up funds are for property acquisition and renovation. $550,000
Integrated Treatment Services Project 5. Four bed adult residential facility to meet the needs of clients with psychiatric and behavioral challenges who currently reside in an SDC, MHRC, or IMD or at risk of being placed into one of these types of facilities. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $150,000
Anka Project 6. Three bed adult residential facility to meet the needs of clients with autism who currently reside in an SDC, MHRC, or IMD or at risk of being placed into one of these types of facilities. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. InAlliance Project 7. The start-up funding is for two (2) clients with $10,000 allotments of funds to aid in the transition of clients from facilities ineligible to receive federal funding into their own homes with supported living services. The funds would be used for assessment of the client, training of the staff for the specific medical and behavioral support needs of the clients, procurement of the residence, furniture and supplies needed to support the client in the community. $20,000

Health and Life Organization (HALO) Project 18. Dental services to meet the unique dental needs of clients who will be transitioning from Sonoma Developmental Center. Start up funds are to assist in the development/construction of a dental office, equipment necessary to serve regional center clients, staff training, etc. $500,000
Brilliant Corners (NPO) Project 19. Due to the closure of the developmental centers and the multitude of projects needing to be developed, it is critical of the NPO have a local presence to oversee the timely development of said projects. Start up funds are to assist the NPO in establishing a local presence including lease of an office space, office supplies/furnishings, etc. $50,000

Project 20. Funding for a joint project with FNRC, RCRC, SARC, NBRC, GGRC, RCEB, to fund CART Model to provide clinical support to our individuals (in person or teleconference/telemedicine), have an advocacy and research branch, as well as training for any clinical students or physicians so develop these resources in our communities and provide a way to maintain these resources. Start up funds are provided to assist the provider in completing comprehensive assessments, community outreach to health professional, etc. in order to better serve regional center clients who are transition from Sonoma Developmental Center with unique health care needs. $50,000
