Encare Project 1. 4-bed adult residential facility to meet the needs of adult clients with psychiatric and behavioral issues currently residing at IMD’s, Sonoma DC, and Porterville DC. The focus is on strong behavioral programming combined with an ability to successfully address psychiatric symptomatology and needs. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $175,000
Eve Manor, Inc. Project 3. 4-bed CCL licensed home for clients living at Sonoma DC with medical and minor behavioral needs with strong emphasis on Sensory needs. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $200,000
Hanif Khan Project 4. 4-bed adult residential facility to meet the needs of clients with psychiatric and behavioral challenges who currently reside in an SDC, MHRC, or IMD. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $200,000
Omelagah, Inc Project 6. 4-bed CCL licensed adult residential facility serving clients with forensic involvement including violent behaviors. Clients may have psychiatric diagnoses. Start up funds are for facility modifications, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this facility. $200,000
UCP Project 7. CCL licensed day program to serve up to 30 clients, some of whom are living or will live in a CPP developed ICF/DD-N, ICF/DD-H, 962 home, and med/beh home. Start up funds are for program modifications, equipment, trainings, client transition, etc. that are specifically related to the needs of the targeted clients population and other enhancements of this program. $125,000
