Planning Services Together
As family members, self-care and
staying healthy are essential to caring for a person with a
developmental disability, along with having the
right supports in place. Alta California Regional Center
(ACRC) clients will have a designated Service Coordinator to
facilitate the planning process. The Service Coordinator, and the
client encompass the Planning Team, which often includes parents,
caregivers, and family members. They work together to review
options and once the options are agreed upon, the Service
Coordinator puts services and supports in place to help meet the
client’s identified goals. Services and supports are
developed by building upon a client’s natural support while
maximizing the use of generic resources, as well as arranging to
purchase identified services and supports. Goals, services and
supports create the Individual Program Plan (IPP), which is
unique to every client. ACRC supports service options that value
clients as productive and contributing members in their
community.
Topics discussed by the Planning Team can include:
- Advocacy and Client’s Rights
- Behavior Services
- Future Planning
- In-Home Respite Services
- Living Options including Residential Placement
- Navigating and Accessing Service Systems
- Out of Home Respite Services
- Parent/Caregiver Education and Conference Attendance
