Draft Survey for Provider Rate Increases

The Department of Developmental Services has released a draft of the survey that all service providers that received the rate increases that went into effect July 1, 2016 as a result of the ABX2-1 legislation must complete.  This law requires that DDS, with regional center participation, conduct a survey of all providers that received this rate increase.  Providers who do not complete the survey by October 1, 2017 will forfeit the increase.

This is just a draft of the survey and no action is required at this time.
