“Thinking Ahead”
My Way, My Choice, My Life at the End

“Thinking Ahead” My Way, My Choice, My Life at the End is about self-determination and choosing preferences including decisions at the end of our lives with an advance directive stating how we want to be cared for. Thus “Thinking Ahead” My Way, My Choice, My Life at the End will assist persons with developmental disabilities to make these very important decisions.

These materials (DVD and Workbook) were developed by a Wellness Grant sponsored by the California State Dept. of Developmental Services.

Partners that participated in developing advanced health care planning for people with developmental disabilities included the California Coalition for Compassionate Care, The Board Resource Center and consumers and care providers at 3 Regional Centers – Alta California Regional Center, Golden Gate Regional Center and East Los Angeles Regional Center along with advocacy  groups.

Focus groups of consumers at the 3 Regional Centers met to develop consumer-friendly materials that use words, symbols and pictures designed by the consumers in the groups to reflect their values, goals, trusts and treatment procedures in an understandable format for end of life care decisions.

The materials were field-tested and revised into the final format. The instructional workbooks and DVDs have been produced in 3 languages: English, Spanish, and Chinese.
